Marquette General Rehabilitation Center

Social workers at the Upper Michigan Rehabilitation Center provide a wide range of services to help patients and their families cope with illness or injury. The staff is trained to help inpatients and outpatients deal with psychosocial issues related to physical rehabilitation.

Social Work Services:

· Patient and family counseling
· Psychosocial assessments
· Education
· Assistance in identifying and obtaining financial assistance
· Private and governmental social services agencies
· Support groups
· Transportation
· Identifying resources and assisting with referrals for: home care needs; skilled nursing and therapy needs; alternative living arrangements including home for the aged, assisted living, adult foster care and nursing homes

Social workers work closely with patients, caregivers and the healthcare team to insure medical, psychological and social needs are met.

Case Management Services

Case managers at the Upper Michigan Rehabilitation Center help to insure quality cost-effective services are provided to patients receiving physical rehabilitation. Case managers can be involved in either inpatient or outpatient programs. Services include:

·	Coordination of the health care treatment team
·	Identify cost-effective patient centered treatment
·	Insure seamless delivery of care between healthcare systems
·	Interface with insurance carriers as needed to provide information necessary to facilitate coverage
·	Physician coordination with physiatry (specialists in physical medicine and rehabilitation) or the patient’s referring physicians as needed
