The Marquette General Health Card is a prepaid primary care services membership card that is accepted at Gwinn-Sawyer Medical Centers, Negaunee Medical Associates and FamilyCare Doctors in Marquette. Designed for people who have no health insurance, or who have high deductibles, the card offers three levels of membership:

· Basic Membership ($125) — Covers an annual physical, with age/gender appropriate lab (physical includes a pap smear and pelvic exam for women; prostate exam for older men; counseling for weight reduction, tobacco use and exercise; appropriate immunizations; and lab work). It also covers one regular office visit, a flu shot for high-risk patients and a self-care guide. Additional memberships are $100 per person.
· Premium Membership ($175) — Includes the Basic Membership, along with an additional regular office visit, and an EKG card or discount on mammograms. Additional Premium memberships are $150 per person.
· Premier Membership ($225) — Includes the Premium Membership, along with one additional regular office visit and a complete Wellness profile. Additional Premier memberships are $200 per person.

Dr. Frederick Hoenke, board certified in family practice, geriatrics and addiction medicine at FamilyCare Doctors, said the card emphasizes Marquette General’s commitment to its patients’ well-being by making available both acute and preventative primary care.

“Locally, it’s a new concept that enhances the convenience of primary care services,” Dr. Hoenke said.

Added MGHS family practice physician Dr. Larry Skendzel, a colleague of Dr. Hoenke at FamilyCare Doctors: "The health card will help fill a gap by providing discounted regular wellness exams for patients, and help those who have high deductibles or are uninsured.”

The annual Health Card is effective 24 hours after payment is received and verified. Cardholders will receive an annual renewal notice at the conclusion of the enrollment period. The Health Card is not an insurance plan, but a supplemental, low-cost opportunity to receive discounts on primary care services.

Enrollment cards/brochures are available at Gwinn-Sawyer Medical Centers, Negaunee Medical Associates and FamilyCare Doctors. An enrollment form can also be downloaded from the MGHS website >> click here.
