Page 2 Different departments of MGHS also may share medical information about
you in order to coordinate the different things needed for your treatment,
such as prescriptions, lab work and x-rays. We also may disclose medical
information about you to people outside MGHS, such as family members, clergy,
or others we use to provide services that are part of your care. Psychotherapy
notes will only be disclosed without your authorization to the person creating
those notes, to those involved in training and quality assurance operations,
and to defend MGHS in an action you might initiate.

Page 3 You have a right to request that your information not be included in the
MGHS directory or restricted to family members, and such request can be
made during the registration process or anytime during your stay in the
hospital, we are required to conform to your request. If you make a request
to restrict such information after the registration process, we are unable
to take back any directory information disclosures that may have already
been made prior to such request.

Page 4 - To report births and deaths; Coroners,
Medical Examiners and Funeral Directors. We may release medical
information to a coroner or medical examiner. This may be necessary,
for example, to identify a deceased person or determine the cause of
death. We may also release medical information to funeral directors as
necessary to carry out their duties.

Page 5 Right to Inspect
and Copy. You have the right to inspect and obtain copies
of your medical information that may be used to make decisions about your
care. Usually, this includes medical and billing records, but does not
include psychotherapy notes. If you wish to inspect and obtain copies of
your medical information that may be used to make decisions about you,
you must submit you request in writing to the Medical Records department
at MGHS. If you request a copy of the information, we may charge a fee
for the costs of copying, mailing or other supplies associated with your
request. We may deny your request to inspect and obtain copies in certain
very limited circumstances. If you are denied access to medical information,
you may request that the denial be reviewed. Another licensed health care
professional chosen by MGHS will review your request and the denial. The
person conducting the review will not be the person who denied your request.
We will comply with the outcome of the review.

Page 6 prevents us from obtaining payment for services provided to you by MGHS.
If we do agree, we will comply with your request unless the information
is needed to provide you emergency treatment. To request restrictions,
you must make your request in writing to the Medical Records department
at MGHS. In your request, you must tell us (1) what information you want
to limit; (2) whether you want to limit our use, disclosure or both; and
(3) to whom you want the limits to apply, for example, disclosures to your
spouse.
