   SUBMIT ONLY 1 OF THE FOLLOWING SPECIMENS:

Plasma
Draw blood in a heparin (green-top) tube(s). 
(PLASMA GEL TUBE IS NOT ACCEPTABLE.) 
Spin down and send 2.0  mL OF heparinized plasma refrigerated.
NOTE:  1.  Indicate plasma on request form.
                  2.  Label specimen appropriately (plasma).
                  3. Trough levels are most reproducible.
 

Serum

Draw blood in a plain, red-top tube(s). (SERUM GEL TUBE
IS NOT ACCEPTABLE.) Spin down and send 2.0 mL of 
serum refrigerated.
NOTE:  1. Indicate serum on request form.
                  2. Label specimen appropriately (serum).
                  3. Trough levels are most reproducible.
