   Draw sufficient plain red-top tubes and send 2.0 mL of serum, or draw
sufficient lavender-top or green-top tubes and send 2.0 mL of EDTA or
heparinized plasma FROZEN in plastic vial on dry ice.
 NOTE:  1. Indicate serum or plasma on request form.
        2. Label specimen appropriately (serum or plasma).
        3. Sample MUST be frozen within 6 hours of draw time.
        4. SST tube is not acceptable.
