IF THE PATIENT'S NAME IS MISSPELLED ON THE SPECIMEN
TUBE, OR IF THERE IS NOT AN IDENTIFICATION NUMBER ON
THE TUBE, IT WILL NOT BE ACCEPTABLE FOR USE.

has left the patient's side)
