     5 - 10 minutes.
2.  Please complete a "Transfusion Service Requisition" form and forward it
     with the specimen. This form is supplied by MGH.

IF THE PATIENT'S NAME IS MISSPELLED ON THE SPECIMEN
TUBE, OR IF THERE IS NOT AN IDENTIFICATION NUMBER ON 
THE TUBE, IT WILL NOT BE ACCEPTABLE FOR USE.
(These items cannot be changed or added once the specimen
has left the patient's side)
