  1 lavender-top (EDTA) tube (minimum: 2.5 mL of EDTA plasma).
Maintain specimen at 37 C for 15 minutes prior to separation of
plasma from cells. Send both the orginal specimen tube and the
tube containing the separated plasma refrigerated or ambient.
NOTE: LABEL SPECIMEN AS PLASMA.

  1 plain, red-top tube (minimum: 3.5 mL of serum).
Maintain specimen at 37 C prior to separation of serum

tube containing the separated serum refrigerated or ambient.  
NOTE: LABEL SPECIMEN SERUM.

IF THE PATIENT'S NAME IS MISSPELLED ON THE SPECIMEN
TUBE, OR IF THERE IS NOT AN IDENTIFICATION NUMBER ON 
THE TUBE, IT WILL NOT BE ACCEPTABLE FOR USE.
(These items cannot be changed or added once the specimen
has left the patient's side)
