Payments

As you visit our office requesting care, you assume responsibility for your bill.  We request payment at the time of service, and accept MasterCard, VISA and Discover cards.

If you would like an itemized statement for insurance, please request it at the time of your visit.  Otherwise, you will receive a statement for "outpatient service" from the Marquette General Hospital Physician Billing Office.

If you need an itemized statement at some time after your office visit, please request it from the Physician Billing Office, which can be reached at 225-4533.

We participate with Blue Cross/Blue Shield, Medicare, Medicaid, CHAMPUS and Tricare.  Other insurances welcome.
