Paid time off is offered to all non-contract employees hired for a position .5 or greater for the purpose of vacation, holiday, religious observances, personal or family business, illness, doctors appointments, or for any other reason deemed appropriate by the employee and approved by the manager.
Employees are eligible for this benefit after having successfully completed the probation period.
Employees
who work an 8-hour shift starting after 12:31 p.m. and before
8:00 p.m. will be eligible for afternoon shift premium.
Employees
who work an 8-hour shift starting after 8:01 p.m. and before
3:01 a.m. will be eligible for night shift premium.
Employees
who work an 8-hour shift beginning with the night shift
on Friday through and including the evening shift on Sunday
will be eligible for weekend shift premium.
A longevity
bonus program was established to recognize those employees
who have an extended period of employment service with MGHS.
Those employees who have been paid for 1,000 hours per anniversary
year for at least 10 years are eligible for a yearly longevity
cash bonus as follows:
* 10 - 14 years: $100
* 15 - 19 years: $200
* 20 - 24 years: $300
* Over 25 years: $400
A year
is an anniversary year in which the employee was paid for
at least 1,000 hours. This benefit is normally paid on the
pay period following the employee's most recent hire date.
Should
you be called to serve on a jury during your regularly scheduled
work time, you will be paid for that day, provided you turn
in the jury duty money paid to you, less travel and reimbursement
expenses, to the Payroll Department.
Should
a death occur in an employee's immediate family, time may
be taken off with pay from scheduled work time during any
of the five calendar days immediately following the death
to attend or make arrangements for the funeral according
to the following:

A form must be filled out and sent to Payroll documenting
the leave within 3 days of returning.
* Full-time employees: 3 work days
* Regular part-time employees: 2 work days
* Irregular part-time employees: 1 work day
EDUCATIONAL
BENEFITS
Employees
have many different programs available to them in educational
benefits. These include tuition reimbursement for courses,
as well as extensive opportunities for in services and conferences.

Any
full-time or regular part-time employee working at least
64 hours per pay period can receive $75.00 (per credit hour),
up to eight credit hours per semester for courses attended.

Conferences (Policy #200-033)
MGHS
holds, sponsors and participates in hundreds of conferences
each year. Many are sponsored by and held at MGHS, while
others may be in the U.P. or available through organizations
outside our area. In addition, Health Services Library and
Education Department have a large inventory of resource
materials, audio and videotapes, and programs covering a
wide range of topics.
Departments
have in services scheduled within their areas, as well as
MGHS-wide education programs set up for all employees.
Employees
are encouraged to participate in the conference and educational
opportunities at all levels.
SOCIAL
SECURITY
In
addition to our retirement plan, MGHS contributes to each
employee's Social Security program in an amount equal to
what you pay as an employee.
WORKER'S
COMPENSATION INSURANCE (Policy #200-035)
Michigan
Hospital Association (MHA) is MGHS's third party administrator
for worker's compensation insurance. If you are injured
on the job, you must:
1. Complete
an unusual occurrence form
2. Tell
your supervisor about the injury
3. Notify
employee health if you require treatment
Seek
emergency treatment if necessary and the unusual occurrence
report can be completed later.

Compensation Commission as required by law.
HEALTH
CARE PLAN (Policy #200-026)
A comprehensive
MGHS hospitalization plan administered by Blue Cross/Blue
Shield of Michigan is provided, including coverage for office
calls and other benefits under a Preferred Provider Organization.
Full-time and regular part-time (.5) employees are
eligible for this benefit.
MGHS
pays a significant portion of the premium for this benefit.

DENTAL
COVERAGE (Policy #200-100)
Dental
coverage, including orthodontics, is provided to eligible
employees. The dental plan pays 75% of the customary fees
for routine preventative services (teeth cleaning, X-rays,
fluoride treatment), as well as 75% of the customary fees
for filling, crowns, root canals, extractions and repairs
to dentures and bridges. The program pays 50% of the customary
fees for replacement of dentures and bridges. The program
also pays 50% of the orthodontics for patients through age
19, to a maximum benefit of $1,500.

A comprehensive
vision insurance program is provided by MGHS. It includes
coverage for exams, frames and lenses or contact lenses
on a yearly basis. Full-time and regular part-time employees

PREMIUM CONTRIBUTION PLAN/FLEXIBLE SPENDING ACCOUNTS

The
Premium Contribution Plan is a voluntary plan which
allows employees to pay their premium contributions for
the health and dental plans, as well as dependent vision
premiums with pre-tax dollars.
Flexible
Spending Accounts - In addition to paying premiums on
a pre-tax basis MGHS also offers flexible spending accounts
to assist employees with paying for certain routine expenses,
increasing disposable income, and gaining tax savings. This
is a voluntary benefit designed to save employees money
by allowing employees to pay for out-of-pocket (uninsured)
medical, dental, vision and hearing care expenses under
a medical reimbursement account and dependent care expenses
under a dependent care reimbursement account, all on a TAX-FREE
basis.

MGHS
provides a retirement plan for employees. The plan does
not require employee contributions. It is fully funded by
MGHS.
To
become vested in the plan, an employee must be paid for
at least 1,000 hours in a calendar year for five years.
Once vested in the plan, you will be eligible to draw benefits
at normal retirement age (age 65). You may draw benefits
after 10 vested years at a reduced amount as early retirement
(age 55 to 65).
The
amount of your retirement benefit is dependent upon the
number of credited years at MGHS and your final average
earnings. Your retirement benefits are in addition to Social
Security benefits for which you may be eligible.
Multiple
options are available in the plan which can tailor the benefit
to your particular needs at the time of retirement. Once
you are vested, a calculation of your retirement benefits
can be requested through the Employee Benefits Office in
the Human Resources Department.

401(k)
PLAN (Policy #200-131)
Retirement Plan (Policy #200-131: Non-RN employees hired 1/1/2006 and all RN employees hired 6/1/07 or later)
MGHS
offers a 401(k) plan to eligible employees matching $1 for
$1 up to a maximum of 4%. Eligible employees who do not
fill out an enrollment form or a waiver of participation
form will be automatically enrolled at 2% contribution into
a balance fund option.
No load
mutual funds and two stability of principal options are
offered through the program. You can view investment options,
balances, and make changes via the Internet at www.pensiontrend.com at 1-888-349-2149. A personalized statement will
be furnished on a quarterly basis depicting the status of
your personal account.
