Your First Appointment

 

Your first scheduled appointment with the diabetes educator will last approximately an hour. This is an individual session and you are welcome to bring along a support person. During this visit the educator will go over your health history and answer any questions you have about diabetes. The educator will work with you on setting reachable goals.

 

Depending on your needs, this may be your only appointment with us or you may choose to come back for follow-up education, or attend one of our group classes.

 

Note: Prior to your first appointment you will need to register with admitting. You can do this 10-15 minutes before your appointment on the 1st floor of the Neldberg building or register online.

 

Your 1st visit includes:
* Health/risk assessment
*Foot exam
*Blood pressure
*Blood sugar meter and instruction
*Teaching

 

We are located on the Ground Floor of the 84 building, near the cafeteria.

 

From skywalk:
Take elevators R,S,T to the Ground Floor. Follow the green line - the diabetes education office will be on your left.

 

From outside:
Enter through the East Enterance and go through the lobby into the hallway. Follow the green line, the diabetes education office is located at the end of the 2nd hallway.

 

 Our appointment hours are Monday-Friday 8:30am-3:30pm.
