Care
Levels at Brookridge Heights
All residents
receiving the three levels of care will be mobile but may
use assistive devices or use a wheelchair and must be able
to feed themselves. If residents need assistance in transferring,
they must only require a one-person transfer.

CARE LEVEL I - Independent ADLs with minimal assist
1.Stand by or minimal hands on assistance with ADL's - bathing,
grooming, and oral care (may need back washed and lotion applied)
- bathing one time per week; may request whirlpool in place of
shower
2. Verbal assistance with clothing selection
3. Stand by or minimal hands on assistance with dressing and undressing
4. Assistance if requested to dining room and facility activities
5. Housekeeping (including bed making), linen and towel service
weekly 6. One well-being check per shift
7. Visual and verbal reminders for meals and activities
8. Laundry service -- 2-3 loads one day per week
9. Medication Administration
10. Standby or minimal hands-on assistance with daily skin treatment
requiring lotions - Treatment or dressing changes once daily for
one week
11. Minimal assistance with self management of incontinence
12. Routine vital signs and/or weights
CARE
LEVEL II - Routine hands-on assistance with ADLs
1. Hands on assistance with meal set-up, bathing, grooming, and
oral care -bathing-up to 2 times per week including one whirlpool
if
requested
2. Hands on assistance with clothing selection and dressing/undressing
3. Assistance to meals and facility activities. Episodic one-person
transfers
4. Housekeeping weekly (daily bed making)
5. Linen and towel changes two times a week
6. Laundry- 1-2 loads up to 2 days a week
7. Two well-being checks per shift
8. Visual and verbal reminders of meals and activities
9. Daily skin care requiring ointments
- Dressing changes for up to two-three weeks. 10. One nightly toileting
check
11. Assistance with self-management of incontinence /toileting
care 6-8 times a month
12. Encouragement to eat and drink daily
13. Other non-routine assistance by staff- up to 15 minutes daily
14. Routine vital signs and/or weights
15. Requires nursing monitoring or intervention I - 2 times per
month
16. Medication administration-including frequent injections
17. Accu-check monitoring of blood sugars 18. Oxygen care
CARE
LEVEL III - daily hands-on assistance with ADLs
1. Hands
on assistance with bathing, grooming and oral care - bathing
up to three times a week, including whirlpool if requested and
partial bathing daily
2. Daily hands on assistance with selection of clothing, dressing
and undressing
3. Assistance to meals and activities
-Continuous one-person transfer and propelling of wheelchair assistance
4. Housekeeping weekly (daily bed making)
5. Multiple linen and towel changes weekly
- Laundry 1 - 2 loads up to three days a week
6. Three well-being checks per shift
7. Visual and verbal reminders of meals and activities
8. Daily skin care requiring lotions or ointments -dressing changes
greater than three weeks
9. Two nightly toileting checks
10. Assistance daily with self-management of incontinence/toiletng
care
11. Encouragement to eat or drink daily
12. Other non-routine assistance by staff up to 15 minutes twice
daily
13. Routine vital signs and/or weights.
14. Requires nursing monitoring or intervention > one time a
week or daily
15. Medication administration -including frequent injections
