Scheduling an Appointment -

In order to schedule an outpatient appointment, please call us at 225-3994 or 1(800) 562-9753, ext. 3994 for pre-registration. 

At the time of your call, we will be asking you for basic information, such as your name, address, phone number, and insurance.  We will also ask about the reason that you are scheduling the appointment and what kind of a clinician (psychologist, therapist, psychiatrist, substance abuse counselor) that you want the appointment with.  If you aren't sure, we will help you figure it out.

After you have scheduled your appointment, click on the Personal Data Form if the appointment is for an adult (18 or older).  If the appointment is for a child or adolescent (under 18), click on the Parental Questionnaire. Complete the form and bring it with you to your first appointment. 

This information will assist your clinician in providing you with a comprehensive behavioral health assessment.
