Why pre-register?

To expedite the registration process in order to decrease patient wait time.
We know your time is valuable!

How is a patient pre-registered?

Please submit this form at least five days prior to your scheduled visit. This allows the staff adequate time to verify and pre-certify your insurance (if applicable). If you are unable to meet this time frame, pre-registration can be completed by telephone the day prior to your outpatient test.

Call the Patient Access at 906-225-3302, or toll free, 1-800-562-9753 ext. 3302, in Michigan or N.E. Wisconsin.

Be prepared to provide the following:

Date of test and procedure
Demographic information
Insurance information (on card)
Complete physician names(s)

Hours for Pre-registration

The Pre-registration Office is open 24 hours a day, 7 days a week so you can call any time.

Will patients be required to stop at registration?

No. By pre-registering you will be able to report directly to the area where you will be having your test/procedure.

Will it be necessary for the patient to bring their insurance card?

Yes. The hospital needs a copy of the insurance card in order to verify coverage and to determine if pre-certification is required.

What is the Medicare Secondary Payor Questionnaire (MSP) and why does it need to be completed for each registration?

Federal Law requires that providers of medical services to Medicare beneficiaries determine whether or not there is other insurance coverage for the beneficiary that should be billed before a bill is submitted to Medicare.

Providers, such as hospitals, must be able to document that they have screened for other insurance coverage by asking the beneficiary questions which will identify whether other insurance is available.

The Medicare Questionnaire contains all of the necessary questions that need to be asked of the beneficiary to determine whether there is other insurance that is primary. Accurately completing the Medicare Secondary Payor Questionnaire ensures prompt payments to providers, as well as compliance with Medicare rules and regulations.

How is a wheelchair obtained when the patient arrives?

Wheelchairs are located at the information desk and/or registration check-in. Patients are encouraged to call the free Valet Service ath the main entrance, and ask for a wheelchar, and patient escort service if needed. The number is 906-361-3365.
